  • First, I will need a written request from you with your legal name at the time of service, the dates of service, and the reason for the request.

    Then, to verify identity, a very brief phone call is scheduled. I include this in the process to ensure the client themself is the person requesting the record.

    Next, a formal release of information and fee agreement is sent to the requester. I only maintain paper files. Therefore, there is a fee for the supplies, etc, required to fulfill the records request.

    Once the copies or scanning is complete and the invoice is paid the records will be sent as chosen by the requester.

    Please note, if there is any reason to believe the records request would not be advisable for a client’s health, therapy notes will be withheld under HIPPA regulation 45 CFR 164.524(a)(1)(i).
